In a video obtained by TMZ, the 36-year-old could be seen dropping his heavy mic to the ground which bounced and struck front-row fan Stephanie Green on the forehead.

The Voice coach could also be seen rushing towards the fan and sincerely apologized before inviting her to join him on stage.
Speaking to the crown, Adam acknowledged: "In my flustered state, I might have thrown my microphone on the ground and then it might have bounced off of the ground, into poor, sweet Stephanie's face... I felt so utterly terribly that I had perhaps wounded this young fan."
Daily Mail reports he later allowed Stephanie and her sister to take a VIP seat on the side of the stage for the remainder of the concert, which was held at the Air Canada Centre.
