The 26-year-old brought out his family to celebrate the Premier League title win during the trophy presentation at Stamford Bridge on Sunday, May 21.
With this Premier League win, Moses becomes the third Nigerian player to win the title.
Nigerian great Kanu Nwankwo won the title twice with Arsenal in 2002 and 2004; current Super Eagles captain Mikel Obi also won it twice with Chelsea in 2010 and 2015.
Moses is aiming for the double this season as he is expected to be in action for Chelsea in the final of the FA Cup on Saturday, May 27.
