Jayawardene, 37, will play no part in Hambantota, though, when the hosts attempt to move just one victory away from securing a series victory over Alastair Cook's side.
Ashan Priyanjan has been called up to replace the former captain after being dropped for the series following his failure to make an impact with the bat in the recent whitewash in India.
A statement on Sri Lanka Cricket's official website said: "Mahela Jayawardene has requested to be excused from the third ODI owing to personal reasons.
"The National Selectors have approved his request and has nominated Ashan Priyanjan to replace him."
