The fourth season of show starred Sharon Ezeamaka as a troubled teenager, Princess, who is struggling to deal with her pregnancy after discovering that she is HIV+.

Emmanuel Ikubese returned as music promoter Femi , who is HIV+. The show also added Adesua Etomi as Sheila, Femi's love interest.
Chris Attoh played the manipulative Nii, while undefinedand Olumide Oworu returned as Tobi and Weki respectively.

The MTV Shuga season focused on relevant issues like stigma, prevention of mother-to-child transmission of HIV, gender based violence, first sex, and disclosure of HIV status.
