Both sides have won all four of their matches, with Real proving too strong for Panathinaikos in an 83-65 triumph.
Pablo Laso's Real made a fast start, jumping to a 20-14 lead at the first change and it was an advantage they extended to 10 points at the main break.
And although Panathinaikos had their moments, they could never make up significant ground on a Real side who added to their lead at every change.
Four Real players scored in double figures, with Felipe Reyes leading the way with 18 points and nine rebounds.
Rudy Fernandez added 15 and seven boards, while Sergio Llull shot 14.
Esteban Batista was the best for the Greek side, with 15 points and nine rebounds.
But Real were far too strong as they maintained their lead at the top of Group E.
Panathinaikos sit third with a 2-2 win-loss record so far.
In Group F, Nando de Colo led CSKA to a big 101-74 win over Unicaja Malaga.
De Colo hit all seven of his field goal attempts, went four from four from the three-point line and also scored both of his free throws in a flawless shooting performance.
The Frenchman's 28 points meant that Malaga could not get near CSKA, with Milos Teodosic, Demetris Nichols and Sasha Kaun all scoring 13 for the victors.
Unicaja remain bottom of the group, without a win.
Elsewhere on Thursday, Alba Berlin beat Galatasaray 75-65 in Group E, while in Group F, Fenerbahce toppled Milan 82-71.
