Why aircraft crashed – AIB

The AIB said further that the aircraft’s crew could not declare an emergency before the crash occurred.

The Accident Investigation Bureau (AIB) has revealed the cause of the Bristow helicopter crash which occurred in Oworonshoki, Lagos on August 12, 2015.

According to the AIB, the helicopter crashed was due to an uncontained engine failure that lasted 12 seconds.

“The crew did not declare an emergency. According to FDR data, the upset lasted for 12 seconds. There were 12 souls on board, including two crew members at the time of the accident,” the AIB’s report, obtained by Punch, read.

“There was no evidence of fire outbreak before and after the crash. The life rafts on the helicopter were observed deflated and floating in the lagoon but were not deployed. The wreckage of the helicopter was confined in a small area around the crash site in the lagoon,” the report added.

One of the victims of the crash was 26-year-old Peter Bello who had just joined Bristow the year before.
